COVID Testing Forms

Our weekly drive-through PCR Covid-19 testing provider will be Pacific Toxicology Laboratories.

Testing is on Tuesdays from January- February from 3:30-5:15pm for students (and for employees). 

If you choose to test off-site, please submit your results to Nurse Tiffany Paulsen.

Below are the instructions and link to register with our Echo Horizon account number. It is very important that when you are registering for the test, please indicate the correct account number in the “entity or organization name” field. You must register by account number and not the name of the school. The results will be uploaded under the wrong account if you use an incorrect account number or list a name.

Registration instructions and link:

Please register for the test no later than 2 days prior to test date. Families who have more than one child at the school should register them separately. Registration should be done under the student’s name, not the parents. We will need the insurance information for each student and staff member when registering for the test. The test requires that patients do not eat or drink 20 minutes prior to swabbing their mouth. Please enter 1939 in the Entity / Organization Name field on the COVID-19 questionnaire link for registration.

Once you have answered all of the required questions, you will receive a confirmation email from Pacific Toxicology Laboratories.  Testing will not take place unless registered for the test. Results will be also emailed to the email address provided at the registration time within approximately 24 hours.

If you already filled out all the forms for previous COVID testing through Pacific Toxicology, then you do not need to do them again, but we do ask that you sign up on link below so we know you are coming.
